About 100 million Americans are dealing with a medical condition lasting more than three months (the definition of “chronic”) and three fourths of them have multiple chronic conditions. Spending on chronic conditions accounts for three quarters of all healthcare costs. One solution to manage the disconnected maze of physicians, hospitals, pharmacies, labs, long term care facilities, et al, is the creation of health information exchanges-of which there are now 57 in the US.
