Senator Michael Bennett (D-Colorado) has introduced a bill titled “The Medicare Care Transition Act of 2009” that calls for a national network of transition “care coaches,” aimed at reducing hospital readmissions, and based on a system Colorado already has in place.  In 2004, Medicare paid out $17.4 billion for rehospitalizations for the elderly.      See full text of Bennett’s bill
