Case managers often do a juggling act between their own responsibilities and those of the emergency department physicians and admitting physicians. In every case, communication impacts the quality of care provided, patient transitions and patient engagement. This piece reviews improvements in case management, conducted by the Huron Consulting Group (Chicago) at the University of Virginia Medical Center—a 570 bed Level 1 Trauma Center, which logs 27,000 inpatient stays annually.
